Any Disabled person who is in need of prosthetic aids and whose family income is below Rs. 400/- per month.
Social
welfare department Jammu and Kashmir is established to safeguard the interests
of poor and downtrodden sections of society i.e. people living below the poverty
line and people belonging to backward classes such as Scheduled Caste (SC),
Other Backward Classes (OBC), Physically Challenged Persons (PCP),
Economically Backward Classes (EBC) and De-notified Tribes (DNT).
Social welfare department launched numerous
schemes for the welfare of poor, distressed and physically Handicapped. One of them
is Grant of Prosthetic Aid for fitting-up/repairing of Artificial Limbs. The details of this scheme are as below:
Scheme Details
Under
Grant of Prosthetic Aid for fitting-up/repairing of Artificial Limbs scheme the costs of the Specific prosthetic aid for
fitting up/repairing of Artificial Limbs will be provided by the Social Welfare
department.
Eligibility Criteria
Any Disabled person who is in need of
prosthetic aids and whose family income is below Rs. 400/- per month.
Documents Required
Duly filled application
form should be accompanied by
§ A certificate on the
prescribed form by the Surgeon/Orthopedic/ENT specialist, countersigned by the
Superintendents of the Hospitals/District Medical Officers indicating the type
of disability and appropriate prosthetic aid required by the applicant.
§ A certificate on the
prescribed form from Revenue Officer not below the rank of Tehsildar or an
affidavit attested by a Gazetted Officer/1st Class Magistrate or a certificate
from a Gazetted Officer of Central/State Government or a Member of the Parliament/State
Legislature indicating the monthly income of the family from all sources.
§ The application forms are
available with the District Social Welfare Officer (DSWO).
PROCESS
On
receipt of application duly completed by the applicant with regard to the
disability and type of prosthetic aid required.
DSWO
shall consolidate the list and forward to the District Level Sanctioning
Committee, comprising of Chief Medical officer as member, DSWO as member
Secretary and Deputy Commissioner as Chairman.
The
Committee authorizes the District Social Welfare Officer to purchase these
aids/fitments from the approved agencies for onward disbursement among the
sanctioned beneficiaries.
